The rich taking advantage of the poor.

Political instability.

Peasant revolts.

The first one may not sound very harmful but it really did reduce the amount of tax income towards the goverment. The rich didnt have to pay taxes because they were large landowners. Farmers would go into debt and had to borrow money from large landowners, who charged very high interest rates. If the farmer couldnt pay the debt back the landowner took possession of the farmers land. The rich kept getting more and more land with out being taxed while the farmers, the source of the governments income were losing it all so they couldnt pay.

What kind of failure do you mean? If you are saying political failure, I think it must be emeprors' failure to hold their power and gave Empress, Prime Minister, War Lords, and eunuchs the chance to control the government. This happened for several times and caused the collapse of monarchic power which led to the end of empire.

What do you mean by downfall? The Han dynasty made many improvements and inventions. The word "downfall" shouldn't even be used to describe the Han dynasty at all.
