i used to have one of these however I upgraded but before doing so I did have to replace the clutch as my lil bro wore mine out I tried a clutch from a rotary Honda that didnt even fit in however I got in touch with Toyota and they sent me a clutch from a 1998 Toyota tacoma 4x4 it worked until it broke and I needed a new one
May have to remove the head for access or to drill them out
In my 2004 Sienna there was a weld in the door assembly that broke and caused a loud clunking noise when I opened the door. Toyota recently issued a recall for this problem and will fix it free of charge

the belt is most likely broke.
Replace the handle. You will need to remove the door panel to replace the handle. If you can't do this yourself take it to the dealer. They should charge you about 1.0 to 2.0 hours labor to do this, plus the part.
its broke
Replace it with a new one.
