The circulatory system contains high pressure vessels called arteries which come from the heart and then the capillaries where gas and nutrient exchange ocurrs. From there veins form and they come back to pick up more. The veins are high volume but low pressure vessels. Arteries are lower volume and higher presure vessels.
See the video below for good information.
The Vena Cava has the lowest pressure out of all the blood vessels.
All the veins, except the pulmonary veins.

In the alveoli, oxygen diffuses into the blood that the capillaries carry due to a concentration gradient that exists there as oxygen conc is higher in the alveolar sacs than in the blood capillaries. Similarly carbon dioxide diffuses out of the blood capillaries into the alveolar sacs down the concentration gradient. The constant flow of blood is there to ensure this concentration gradient is established at all times.
The large artery connected to the heart, called the Aorta.
At the level of the capillaries, oxygen will diffuse out of the saturated red blood cells down their concentration gradient into the tissues where their concentration is lowest.
Pulmonary arterial system has high pressure but low oxygen concentration.

The lack of nutrients inside a cell compared to the blood vessel creates a concentration gradient between the blood vessel and the cell. Due to the lower concentration in the cell, the nutrients diffuse through the blood vessel wall and into the cell.
No, a Capillary is a small blood vessel that delivers oxygen and other nutrients to cells.
Oxygen and other gases pass through the capillaries.
Oxygen diffuses into the blood in the lungs and binds to the hemoglobin since the oxygen concentration is high and the carbon dioxide concentration is low. The blood is pumped to the body. The hemoglobin releases the oxygen to the tissues because here, the concentration of oxygen is low and that of carbon dioxide is high.
