I can't find much definitive information, but it appears that no living amphibians are herbivores in their adult forms.
Most amphibians are at least partially herbivorous as larvae.
The only ambiguous exception is a species of caecilian, Afrocaecilia taitana. One study that examined the stomach contents of these animals suggested that they ate plant material, but some experts say that the stomach contents were simply the remains of earthworms, and the plant material that the worms had eaten. Caecilians, generally, are little known in any case.
