To my knowledge, through 2013, there are only two men who have accomplished this: Bobby Knight and Billy Donovan.
Bobby Knight was a player on the Ohio State teams that made it to the NCAA men's Basketball tournament Final Four three consecutive years 1960-62, winning the title in 1960. He then coached Indiana to the Final Four five times (1973, 1976, 1981, 1987 and 1992), wining the championship in 1976, 1981 and 1987).
Billy Donovan played for Providence when that school went to the Final Four in 1987. He subsequently coached Florida to the Final Four in 2000, 2006 and 2007, winning the title in 2006 and 2007.

The University of Kentucky last won the NCAA Men's Basketball Championship in 1998. Tubby Smith was the head coach and the tournament's Most Outstanding Player was Jeff Sheppard.
