Hannah appears only once in the TV series the Walking Dead. The character is explored in more depth on the webisodes.
In the pilot episode, Sheriff Rick Grimes encounters what is left of Hannah crawling across the park near his home, and humanely puts her out of her misery.
This is, incidentally, one of the first illustrations of Rick's compassion for the walkers that is later explored in the fall finale, episode 7 of season 2, "Pretty Much Dead Already."
