The worst Jedi.... I would say the worst Jedi is a padawan without a master. I cannot place any names though. The reason the worst Jedi is a padawan without a master is because even with a master a padawan can be easily influenced. Without a master, they would have to fend, teach, and look out for themselves. At first a young padawan is recklace and almost willing to do anything. If this is left alone, the padawan will grow around this. Even if they teach themselves properly and correctly, the Sith would be targeting them. So, it would be very unwise to be a padawan without a master. With this, they would grow and be the worst Jedi. That is why Jedi are careful for these kinds of people. But to my knowledge, there is no record or names.

I would answer with yes - absolutely. Once he took on the title "Darth" and became an apprentice to Darth Sidious, he became a Sith Lord. Even the apprentices in the Rule of Two created by Darth Bane are Sith Lords. If you are a Sith after the time of Darth Bane - then you are a Sith Lord whether you are the master or the apprentice. And before Caedus (Caedus is the correct spelling) there was Dark Lady of the Sith Lumiya who converted Jacen Solo to the dark side.
