If the vice president becomes president, he nominates a new vice president who is sworn in after a confirmation vote by a majority of both houses of congress. The same is true if the vice president dies or cannot continue as vice president -- the president nominates a new vice president.

Under current law (specifically the 25th Ammendment), when the office of Vice President is vacated, the President appoints a new Vice President. Both houses of Congress must approve the new Vice President by a majority vote.

When Spiro Agnew resigned as Vice President, then-President Richard Nixon appointed Gerald Ford as the new Vice President. When Nixon subsequently resigned, Ford became President. Mr. Ford is the only President to assume that office without being elected to it.
