Craig MacTavish, current GM of the Oilers.
I have no idea who the last five players were, but the very last one was Craig MacTavish, who is the current head coach of the Edmonton Oilers. His last season was with the St. Louis Blues in 1996/97. When he retired at the end of that season he was the last player to not wear a helmet.
The 1979 NHL Draft class was the first group of players who had to wear a protective helmet. Veteran players, those in the league prior to 1979 were able to continue playing without helmets. The last player in the league to play under the "helmetless grandfather clause" was Craig MacTavish in 1997.
Craig MacTavish was the last "bare-headed" NHL player. He retired after the 1996-97 season and is currently the head coach of the Edmonton Oilers.

Willi Plett, Brad Smith, Guy Lafleur
Bob Montgomery. Montgomery was a catcher for the Boston Red Sox between 1970-1979. The rule making helmets mandatory was made in 1971. Those players that played on an MLB roster prior to 1971 were not made to wear a helmet while batting if they chose not to do so. Montgomery didn't and played his entire career without wearing one. Montgomery's last plate appearance came on September 8, 1979 in a game against the Baltimore Orioles.
1966-67 was the last season in which the Toronto Maple Leafs won the Stanley Cup.
In 1988/1989 season, Ron Dugay was still not wearing a helmet. I believe he was the last King player to do so. The next year Dugay played for the San Diego Gulls so if it was not him it would have to be after that season.

Inheritor, the last rank in the game. The last helmet, the Haunted Helmet, is the Pilot Helmet with a skull on it. To unlock it, you must reach the rank Inheritor, and own every BASE helmet in the game.

If a biker was to come off his motorbike, the last thing he'd want would be for his skull to hit the same material as the exterior of the helmet. Without this lining the helmet is abit pointless. The lining acts as a sponge between the skull and the helmet. On impact alot of the energy, exerted by the ground or object which hits the helmet, is dissapated by the inner lining and cushion inside… Read More

It depends on what team's helmet you are reffering to. All NFL helmets have the following decals in the back: American flag, NFL logo and the manufacturer's safety warning decal that came with the helmet. Many teams also have the player's number on the back. The Dallas Cowboys have the last name of the player on the back of the helmet courtesy of a household labelmaker.

Helmets became mandatory in the NHL in August of 1979. The only players exempt from the rule were those who signed a professional contract prior to June 1, 1979, and also signed a waiver allowing them to play without a helmet. There are 2 skull helmets in reach: One is unlocked at commander and costs 120,000 cR and the other is one of the add ones for a helmet and is pretty expensive as its the last helmet.
The helmet with the skull in it is the last available helmet in the Halo Reach armory. You must first purchase ALL the other remaining helmets before buying the skull helmet, which also requires you to be of high rank and have TONS of credit.
