One general reason is that none of the managers of that company choose to go through the steps necessary to document and terminate such an employee. Often there are other reasons that the person is useful to the corporation and those reasons may seem less important. However, in my opinion, there is a huge loss when other knowledgeable employees voluntarily leave the company because of the impossibility of doing their job well with this person. Examples would be that the person stonewalls efforts, is verbally abusive, does not forward work in a timely manner and, I imagine, consistent complaints to the manager regarding this person.
