Puja is what the Hindus call a prayer . Its not exactly a prayer but it bends mostly towards a prayer . Puja is mostly recited by a Hindu priest called a Pandit . There are puj's which are meant for God and for the devil too . Few are " believed " to spread good other to spread evil . If a hindu seems to have some trouble be it social ,economical they consult with the Pundits and the Pundit tell em what kind of Puja needs to be done . Thus its an important ritual and an Hindu performs or comes across Puja many times i his/her life time

One can worship the deity even without the puja items. But if one has time and would like to follow a daily ritual, then they use the puja items. These can be few or many as the situation permits. The puja items are thoroughly cleaned before use.

This depends on the Hindu in question. Some Hindus don't think of any symbols as important, some think of all symbols as important, some think of certain symbols as important and others as unimportant. Usually the list of important symbols would be the swastika and the Om. Just as Cross is important to christians and the crescent and star are important to muslims. Swastika for hindus denotes welfare of the whole world and Om stands for the Supreme spirit in the world.
