Extra-linguistic elements usually come out of the context that the utterance is made in. Context adds extra meaning beyond just the sounds, morphemes, syntax, and other physical elements of the language you're using. Think of it like you have a physical something that is given greater meaning because of what's going on in the environment that surrounds that physical thing. Does that help?
