Velocity (FPS) going through the flush valve or stop valve is excessive
I cured mine by sticking a washer on the cap of the filler then putting lid on. Test it by putting pressure with your finger on the cap and flushing - it worked for me. Needs washer blue tack and hands is all
you probably have a seal in your flush valve assembly that is shot and not closing off the water when the toilet fills back up. Or you have something like the chain if an older assembly sitting under the flapper keeping it from closing all the way. If you're a DYI person it would be best just to replace the assembly, it's pretty easy to do ... plus if it is the assembly, replacing it will save you on your water bill too ...

All toilet tissue is supposed to flush easily in reasonable quantities, NOT in huge clumps
You will probably clog your toilet or pipes if you flush anything but toilet paper. I would not recommend trying to flush underwear down the toilet.
