It's seperate to the original Red Dead Redemption. You can tell straight away as John Marston is still alive.

No there isn't any sexual themes in this game. I own undead nightmare myself and there is no nudity , gambling or scary parts. There is violence obviously because its the aim of the game ; stay alive for aslong as possible and complete it! It is rated 18 in the UK but 15 in Australia. Personally it should of been rated 15 in the UK. Alot of kids have this game and they enjoy it alot!
