Because there is no heat in either front or back, I would suspect the engine thermostat first. If it never opens, the coolant is essentially prevented from getting to the heater cores.

if there is 1 it would be back on the liftgate, but they usually are the same one as the front washers. there is just two lines that separate one for the front and one for the back.
No separate container to fill for the rear. The front washer solvent reservoir serves both front and rear sprayers.
On a 1998 Windstar the vacuum reservoir tank is under the hood in the back left of the engine compartment when standing in front of the vehicle. Mine has a red and black vacuum hose coming out of it.

It sounds as if there is a problem with the rear electronics module in the Ford Windstar. This controls the front and rear lights and it is located in the back in the cubby with the jack.
