Jesus not only taught his disciples through lectures, but he also taught them by forming personal relationships with them. He wanted them to know him relationally not just intellectually as stated in John 14:6-9.

Jesus told His followers to do this so that they will remember what He did:

Luke 22:19 And He took bread, gave thanks and broke it, and gave it to them, saying, "This is My body which is given for you; do this in remembrance of Me."

This is reiterated by Paul in Corinthians:

1Co 11:25 In the same manner He also took the cup after supper, saying, "This cup is the new covenant in My blood. This do, as often as you drink it, in remembrance of Me."

1Co 11:26 For as often as you eat this bread and drink this cup, you proclaim the Lord's death till He comes.

There is no special significance in taking bread and wine it is a form of remembrance - we forget very quickly.

Jesus began a new annual tradition for disciples to follow at Passover. He used bread to represent His body which would soon be broken and wine to represent His shedding of His blood to wash away the sins of mankind.
