The first Vice President to succeed to the office was John Tyler, when William Henry Harrison (the 9th US President) died on April 4, 1841, just 1 month after being inaugurated.

Abraham Lincoln was the first President to be assassinated and was succeeded by Andrew Johnson, who was extremely unpopular and was the first President to go through impeachment proceedings, failing to be convicted (and removed from office) by one vote in the U.S. Senate.

Johnson was not the first Vice President to take office upon the death of the elected President. In 1841, John Tyler (of 'Tippecanoe and Tyler too' fame) assumed the office of President after William Henry Harrison died after only one month in office, due to a probable case of pneumonia brought on (according to legend) by extended outdoors exposure during his presidential inauguration celebration.
