It really depends on the size of the tear. There are patches made out of denim which may be ironed on that are easily found. You could also make your own cute patch out of a favorite fabric, back it with heat and bond "light", iron it on the outside of the jeans then sew around the edges with regular thread or a decrative thread. If you use the regular heat and bond or extra hold, you will not need to sew the edges as it is strong enough to stay in place without stitching, plus the needle will gum up if you try to sew through it. Use your imagination and have fun.

This site has information about a company that will actually repair the hole, not sew or patch it. I guess they somehow replace the fibers. You mail them the jeans and they fix 'em and send 'em back to you.

If you have a "free arm" style sewing machine: pin the patch in place; expose the free arm; slip the leg of the jeans over the arm up to the patch (from the hem); you'll probably be able to sew half the patch in place; then remove the jeans from the arm; then, slip the leg of the jeans over the arm down to the patch (from the waist); you should be able to finish sewing the patch. If you do not have a "free arm" sewing machine, you'll probably have to sew the patch in place by hand. Use medium size stitches and go around the perimeter of the patch twice (in the event one or two of your stitches come open, the second row of stitches will remain to hold the patch in place). Then, depending on how securely you want the patch attached, run another row or two of stitches about 1/2 inch in from the perimeter.
