How do you change Rotom into the other forms?

To change rotoms form in Pokemon diamond go to the galactic HQ in Eterna city. When you're inside go to the right and you'll find a locked door.To open the door use the key you get from the mystery gift event.

On platinum, you need a mystery gift which is over, then you go to team galactics hideout and in the 1st room is a wall with a yellow and black checkered pattern. With rotom in your party, press A while facing it, and it opens. Press A at a one of the machines and he will change form. If you don't have that, trade it onto SoulSilver or HeartGold. Put him as your first Pokemon and go to saffron city. Go to SilphCo. and you can use the elevator. Go in it and, like platinum, there are a heap of machines.
