You can only ever Tribute something that you control, unless specifically instructed to do otherwise. Cards like Lava Golem and Soul Exchange for example, are cards that specifically say the opponent's monster can be tributed.

No. The text has been changed but the effect remains the same, first it was a 'cannot be special summoned except by..' monster, which means only the listed method could ever be used, it was not the kind that could be brought back assuming you summoned it properly first. Now it says "Must be summoned by....cannot be summoned in any other way", again, it doesn't matter if you properly summoned it, it can't be summoned back from the graveyard.

There's no hard limit. Each of your monsters can attack once unless they've got effects that let them declare more, however as long as you can keep replacing your monsters with new ones without ending the battle phase, these can all declare attacks too.

A 'tribute' is when you send one monster from your side of the field to the graveyard, as a cost for a summon, or a card effect.

For example, to Normal Summon a level 5 or 6 monster, you must tribute summon it by sending on of your monsters to the graveyard. Level 7+ monsters need two tributes, unless specified otherwise.

Note that unless specifically said otherwise, a tribute can only be something you control, ie, on your side of the field. It depends on what you mean. There are many cards with a low ATK, but hig DEF, such as Millenium Shield (0 ATK/ 3000 DEF/ 5 stars) and Soul Tiger (0 ATK/ 2100 DEF/ 4 stars). There are a few cards with 0 ATK and DEF, such as Batteryman AA and Chaos Necromancer, that start out with 0 ATK, but get stronger very easily. And there are the low ATK monsters that gain power when a ceratin card is equipped to them. The two biggest combos are Koitsu + Aistu (Aitsu has 3000 ATK) and Doitsu + Soitsu (Soitsu has 2500 ATK). But there are the 0 ATK/DEF monsters that will always stay that way. So I would say that the weakest yugioh card is Cloudian - Smoke Ball. It is a normal LV 1 monster with 200 ATK and 600 DEF. It is practically useless, unless you know how to use it.

Never in the anime *sadly*, however in the manga they pleasure themselves to each other *and other duelist* quite often. Anime.net even made a short 13 episode saga based on the anime, *with BEAUTIFUL animated pictures*.
