Yes there is a F.G.D. Card it does exist, but it is mainly for collection. Hardly will you ever find use of it in a duel or it allowed in a tournament.

The FGD can be summoned with any dragon as long as you have 5.
There is really no specific "dragon gods" the five god dragon is the FGD card itself.

Yes, if you control a monster in defense mode and it is changed to attack mode during the Battle Phase by a card effect, it can attack. The "Zero Gravity" Trap card is an example of a card that can switch your monsters' battle position during the Battle Phase. But if the monster already attacked that turn, was switched to defense mode, then changed to attack mode again by an effect, it cannot attack again on that same turn.

From my experience dueling there has been a few machine synchros that stood above the others.

#1 Ally of Justice Catastor

reason: Easy to synchro, can destroy certain decks with it's effect plus has a fair attack for a level 5 monster.

#2 Power-Tool Dragon

Reason: Once synchro you can obtain a random assortment of equip cards you might have in your deck, invincible if it has at least one equip card equipped, fair attack strenght.

#3 Armory Arm

Reason: Well though Armory arm is weak for a synchro it's effect makes it worth it since it can equip it's self to an other monster so it gains 1000 attack plus any monster destroy by a monster equipped by armory arm damages it's controller's life points equal to it's attack.

#4 Genex Ally Triforce / Genex Ally Triarm

Now Triforce and Triarm can cause havoc cause of their great assortment of effects that can make either fit into almost any deck depending on the monsters used to synchro for them.

#5 Ally of justice light gazer

For me it deserved spot #5 cause it is easy to synchro plus it has a fairly high attack that gets higher if you opponents has light monsters in his graveyard.

The monster card with the highest possible defence without any field spell cards or equip cards is The Wicked Avatar, but only when played against The Winged Dragon Of Ra (Official), whose attack and defence is equal to the number of life points given up by the controller while keeping 100 life points. As there are many ways to increase you life points it is possible for Ra to have upwards of 10,000 defence points, but with The Wicked Avatar's ability it will always have 100 more attack and defence points than any other monster on the field. The next highest possible defence I believe is 10,000 points by playing The Wicked Eraser while your opponent has 10 cards (5 monsters & 5 spell/trap cards) on their field, I think. The highest base defence without factoring in an ability is probably a tie between Dragon Master Knight, Five-Headed Dragon(the card version of FDG which went into print and also featured in Yu-gi-oh season 3) and Malefic Truth Dragon, which each have 5000 base defence. I think the highest base defence for a level 4 or lower monster is Big Shield Gardna who has 2600 base defence points, but I cannot confirm this. So I guess the gods win again after all (Ra = Egyptian God, Eraser & Avatar = Wicked God)

And for the record there are many cards that can either survive multiple attacks or survive being destroyed by battle or card effects indeffinitely. For example, Shield Wing can survive being attacked twice (i.e. it takes three attacks to destroy) and Five-Headed Dragon cannot be destroyed by a Dark, Wind, Fire, Water or Earth monster indeffinitely. Cards like Ultimate Axon Kicker and Red Nova Dragon cannot be destroyed by card effects(eg the effect of Man Eater Bug or Fissure) indeffinitely. Marshmallon (spelt like that because it is a marshmallow, not a mellon from mars) is only one examply of a monster that cannot be destroyed by battle, and any card equiped with the equip card Mist Body cannot be destroyed by battle either.
