I read that when the Presidential Succession Act was drafted in the late 1940s, President Truman thought that elected officials should be at the top of the list rather than presidential appointees. Ironically, about 20 years later the Constitution was amended giving the president the power to appoint a vice president in the event of a mid-term vacancy in the office. However, vice-presidential nominees in this case require the approval of both Houses of Congress, while Cabinet nominees require the approval of only the Senate.

In the same article I remember reading that the reason that the House Speaker is listed ahead of the Senate President Pro Tempore is that either Truman or the author of the bill did not like the person who was Senate President Pro Tempore at that time.
