Upper Deck Authenticated (UDA) items have an attached hologram with a unique ID number for each item signed. If you visit the signed memorabilia data base you could entered the hologram ID number and the information for that piece will be brought up. See Related Links below for the Data Base page.
Every piece of signed memorabilia is packaged with a registration card. (ticket?) If you lost your certificate or card you should be able to contact Upper-Deck and ask for another to replace the one you lost. I don't know their policy or procedure but they can be contacted by phone to assist you.

I did a quick check on eBay and found two jerseys (signed on the front) with Non Hologram authenticity with both selling for $137. One had a COA from Scoreboard inc. a very popular authenticator on items being sold at the Home Shopping Network in the 1990's, and is no longer in business. Another Jersey (signed on the back number 8) with a Major League Baseball Hologram sold for $255.
If you visit the MLB website they are being sold for $639.99 with the Upper Deck Authentic Hologram, and other retail websites sell them for about $400.-$500.

I have not been able to find a similar product to compare with. However, I did find on sportsartifacts.com a 1944 Sacramento Solons Team Signed Baseball with 22 signatures and a certificate of authenticity going for $195 and a 1966 Portland Beavers signed baseball with 15 signatures and a certificate of authenticity going for $50.
Yadier Molina signed baseballs w/ certificate of authenticity are valued around $100 retail.
Depends who signed it, and if there is a certificate of authenticity or not. You could either put it on eBay or enter it in a sports memoribilia auction and see what the market is prepared to pay for it.
A signature is not in itself a legal document; it does not agree to anything or assert anything. It is, at most, a souvenir. Sometimes one can get a certificate of authenticity for something such as a signed baseball, a signed photograph, etc., but that is not the same as notarization.
