Typically it's 7 yards. So if the line of scrimmage is on the 35 yard line then you add 7 yards for the kickers depth behind the line and then 10 yards for the depth of the endzone totaling in a 52 yard field goal.
It is not known how far exactly NFL kickers stand from the line of scrimmage when attempting a field goal. Most NFL kickers average a field goal range of 35 yards.

The ball must be placed on the line of scrimage before snapped. Assuming you are referring to a place kick or a punt, there is no rule dictating where the ball must be snapped to. Seven yards is standard as it should give the snapper, holder and kicker sufficient time to perform the kick.
In the NFL, the ball will be placed at the spot of the hold after a missed field goal. If the kick took place from inside the 20, the ball is moved out to the 20. In college football, the opposing team gets the ball from the original line of scrimmage.
A college kicker uses a one inch block (which has a flat surface and is just used to get the ball higher off the ground) to kick extra points and field goals, but the ball stila has to be held by another player. A kicker does use a tee for kickoffs.
