You should train by running for long distances. To be a midfielder, you have to have a lot of endurance and you have to be smart on the field. To be a great midfielder, try watching some pro midfielders on tv to watch when they run up to help the forwards and when they run back to help the defense. Just put your mind to it!
If they play a full match they would probably be running for about 70 minutes out of the 90. A goalkeeper would be far far less.

As far as they are prepared to go, bearing in mind the run back when defending... and of course who the coach is! Generally, a full back should have their respective midfielder and/or winger in front of them unless running on the overlap or on the break; staying out of position can confuse the team structure and lead to confusion on the defense, which leads to a lot of goals being conceded.
