Possession is turned over once the disc hits the ground- whether it be dropped, knocked down, or thrown to the ground by anybody on the pitch. Also, the disc can be turned over if someone on the defensive team catches the disc.
There are two common ways to defend in ultimate frisbee. The first is to pick a man and defend that man (everyone picks their own man to follow). You can do that or play "zone" where two or three players will take sections of the field and block players from running in or discs from going through. This style is also assocaited with a "cup" where 3-4 people will build a broad wall around the disc thrower (handler) and sidestep to block players from cutting in or discs from flying out.

It's actually just called Ultimate. Frisbee is just a brand name for the "discs" used in the sport of ultimate. So i guess they just decided to call it ultimate because it was so awesome.
