That was pitcher Jim Derrington of the Chicago White Sox who played his first game at age 16 years, 306 days old when he pitched on September 30, 1956 (born November 29, 1939).

Derrington's career consisted of that game and 20 games in the 1957 season. He pitched his final MLB game on September 29, 1957 ... two months shy of his 18th birthday.
