Dental insurance varies greatly depending on the insurer and the policy you choose. Before purchasing insurance you should think about the extent of the work you need done and then get a policy that will cover it all. Many policies have high deductibles and low yearly or lifetime limits on reimbursement so you really must know what you need.

There is no product called cometic dental insurance. Dental insurance generally excludes any procedure that is done for costmetic reasons. That said, crowns are not usually cosmetic but to restore the function of the tooth and would be covered. In most dental plans, crowns are covered at 50%. So you will pay half of the cost of the procedure at the discounted rate negotiated by the insurer with the dentist.

The exception to payment for cosmetic procedures are discount dental plans which get you a discount on all procedures. The difference is that in a discount plan you pay all of the cost; the plan pays nothing.

Ordinarily, the insurer makes payment directly to the dentist, especially if it is a managed care type of claim. Therefore, the time limit for recovering an overpayment will be specified in the contract between the insurer and the provider.

However, if the insurer reimbursed you after you paid the dentist, your insurance contract may specify the process and time limit for the recovery of overpayment. If it does not, the law of the state in which you reside, or where the policy was issued, will specify a Statute of Limitation for breach of contract claims.
