It has a timing chain.
If the truck is equipped with a V6 it will most likely have a timing belt. However if it has the 4 cylinder it will have a timing chain.
V6 = Belt 4 cyl = chain
try changing both timing gears
with a wrench or socket
There are marks on the chain that coordinate with marks on the timing gear. Make sure these line up when putting on the timing chain, and you should be good to go. Getting to the timing chain is the real battle.

I would suspect you need a new timing chain or belt depending on which is installed. A worn belt or chain will jump time.
Sounds like the timing chain adjuster is worn out. It is actuated by oil pressure. Was it replaced with the timing chain?
