That completely depends on the department. All departments require you to be between the ages of 21-35 to get hired and you need to have at least a high school diploma. Most departments these days require at least 60 college credit hours. There are other qualifications such as no felony convictions, no domestic convictions, etc.
In most cases a high school diploma or GED equivalent is sufficient. Police departments are looking for the best qualified candidate. So having college credits is a + but not a requirement in most departments. Some require "X" amount of college credits and some require at least a 2 year degree. No departments that I know of require a 4 year degree. All federal departments i.e. FBI DEA CIA NSA ATF USM require a 4 year degree.
The educational requirements for jobs in the billing departments of hospitals may vary from hospital to hospital. Some may only require previous experience, while others may require an accounting degree of some sort.

This is completely dependent on which department you apply to. Many departments only require a high school diploma (or GED), along with Law Enforcement Certification. The best advice I can give you is to contact a local department (or look it up on their website) what their requirements are. Regulations differ from state-to-state and department-to-department. At minimum, you will require a GED and Law Enforcement Certification (which is earned at the academy). Some departments (usually larger departments) may require some college credits.
Most plans do indeed have basic hospital expense coverage. Some plans may require a deductible or co-pay.
This completely depends on the Fire Department. A lot of Fire Departments in the United States require that their firefighters are EMT-Paramedics. While some only require First Responder training. Some Fire Departments also have ambulances staffed by EMTs and Paramedics that can transport patients to the hospital.
