An online cheque book request cannot be placed via phone. To place an online request you need to have a Bank of Baroda bank account and also have an internet banking login id and password. Once you login to your account, go to other services and then request a cheque book. If you don't have an online banking account then go to their website or check your passbook and then call up their customer care hotline and request for a cheque book.

Yes - If you have your bank passbook and cheque book (or just the cheque book) you can withdraw money from any branch of SBI. If you don't have these, then the bank would not accept your request for money withdrawal. If you have only your passbook, you can try but the bank is mandated to accept your request.
I am not sure if this is possible but you can try it out: a. Call up customer care and ask them for a cheque book b. Go to an axis bank ATM and select cheque book request Most private banks in India offer these 2 facilities for getting a cheque book and I guess Axis bank too has this option
