I just replaced mine at 213000 miles and it would have had many more but i had an oil pump failure. it had very little cylinder wear but could have used a new timing chain.
im not familer much with the older Chevy trucks, im assuming it has a 350 in it and that's a small block a 454 is a bigblock, they have different transmission mounts so you would have to get new trans missin and after thay you need to determine if you need a new driveshaft or you can sometimes cut the one you have YES A 454 WILL FIT WITHOUT MODIFICATIONS,YOU WILL NEED A BIGGER RADIATOR
