Yes, you can. I doubt if this happens often, since the two problems are completely unrelated - but they can cause similar pain - ruling out one, should always include checking the other. Often times a doctor doing abdominal surgery unrelated to the appendix, will remove the appendix too. This way the patient doesn't end up in the ER with belly pain and a scar in the general area of an appendectomy, causing confusion over whether the appendix is the problem.

The symptoms of appendicitis can include:

  • Dull or sharp pain anywhere in the upper or lower abdomen, back, or rectum
  • Nausea and/or vomiting soon after abdominal pain begins
  • Severe cramps
  • Painful urination or trouble urinating
  • Constipation or diarrhea with gas
  • Abdominal swelling
  • Loss of appetite
  • Fever of 99° F to 102° F

Yes. Torsion of the ovarian cyst on right side and acute appendicitis may give you lot of confusion in diagnosis. Both needs surgical correction. Ultrasound examination will help you lot is such situation.
