You can, it is super strict, can be expensive and they only take a small number a year, it a well rehearsed 'show' in the North. You don't really get to see the true North Korea.
You can also go on a DMZ trip, pretty much an all day trip to the border. Again, this is really strict but is a great day out! You can see the famous tunnel made by the North as well as the road of the famous 'axe murders'. You need a passport to go on this trip - but it's great!!

No people are not allowed to leave North Korea, they can escape to South Korea, but risk being shot, or if caught trying to escape North Korea they can get heavy punishment, so the only way to get out of North Korea if your North Korean is to escape out of North Korea secretly, and go to South Korea, via another country that can give you asylum.
The North Korean people are not allowed to leave the country because North Korea is a dictatorship. The government exercises total rule over it's people. There are harsh punishments for anyone who tries to leave the country.
North Korea has more of a communist government, and North Korea has a different leader. He is more of a dictator, because he does not let anyone enter/leave the country, or let anyone badmouth the government.
North Korea has a Juche government. And this type of government tends to have absolute control over it's citizens. Though the fact that they are not allowed to leave the country does not mean it has not seen it's share of attempts to cross south of the border.Being a Juche government, They are told what they can and can not do, say or even wear! So it's a given that they are told they can not leave their country. After Kim Jong-il took over North Korea, he stated that he was never listened to or treated right in that country, so he said that he would treat the People of North Korea the way they treated him. And telling the country that they are unable to leave,gives him somewhat justice I suppose.
