Combining the two can increase the risk of seizures as well as central nervous system and respiratory depression. What that means: the drugs are very similar so taking both is a like taking more of either one. It can cause seizures or even make you stop breathing or go into a coma. The drugs can be taken together if the dosage is correct but you need to know what the correct combined dosage is. Basically Tramadol seems to be safer to use for long term treatment of minor pain (like back troubles) but it may be too soon to be sure how safe it really is. Both drugs are habit forming and there are withdrawl symptoms. There are other drugs that Tramadol and/or codeine react badly with that I have not mentioned. BE CAREFUL. Concomitant use of tramadol hydrochloride increases the seizure risk in patients taking: • Selective serotonin reuptake inhibitors (SSRI antidepressants or anoretics), • Tricyclic antidepressants (TCAs), and other tricyclic compounds (e.g., cyclobenzaprine, promethazine, etc.), or • Other opioids. Tramadol is a very strong pain killers and I would not recommend you take it with Codeine or Paracetamol as Tramadol already contains Paracetamol and will increase your risk of experiencing unpleasant side effects. I would recommend you continue taking your Paracetamol or Codeine every 4-6 hours as prescribed and take your last dose before bed. The next day, begin taking Tramadol but at a low dose, for example only 1 pill because Tramadol can also have some unpleasant side effects. If your pain is very severe, you should contact your Doctor, particularly if your pain relief is not working and/or the pain came on suddenly.

I'm unaware of any formulation which contains both tramadol and codeine, nor can I think of any real benefit to doing so. They're both opioid analgesics and do pretty much the same thing, so there's little point in combining them.
