A CPT-4 code consists of 5 digits unless used with a modifier or when used in the time spent with a physician. For example, if a patient spends 2 hours and 14 minutes with his physician you would code 99354 X 1, 99355 X 2. 99354 X 1 is for the first 30 to 74 minutes and the 99355 X 2 is coded for each additional 30 minutes. Thus equaling a 134 minute visit. For codes used with a modifier you would first code the 5-digit code then the 2-digit modifier. Example; 43820-62. (Don't forget the hyphen between the 5-digit code and the 2-digit modifier.
