No. He nearly died of it when he was a baby and there was a plague outbreak in Stratford. However, there was no plague outbreak in Stratford in April of 1616 which means that Shakespeare died of something else.

Some people have speculated that illness was the cause of Shakespeare's retirement from the theatre world at the age of 49 and his death three years later. But since there is no evidence either way, we will never know.

We don't know exactly what caused Shakespeare's death but it was unlikely to have been plague. Plague was extremely infectious and had a tendency to go epidemic. Had plague been the cause of his death there would have been a record of a plague outbreak somewhere nearby. There wasn't so it is improbable that he died of plague.

There is no evidence that William Shakespeare had any mental illnesses, although some have speculated that he had to retire early from the theatre business due to the onset of Alzheimer's disease.

No, but he nearly caught it when he was a baby. Fortunately he did not. At least two of his siblings (his sister Anne and his brother Edmund) died of it.

No doubt Shakespeare had some medical conditions but we do not have any of his medical records or even any gossipy letters so we are left to guess at what health problems he may have had.
