Well, Ichigo loves Orihime even though she is kinda mean to him. So I really do think he's loves her the best.

Rukia and Ichigo never kiss, they might they might not. I'm secretly hoping for Ichihime but that's just me. Kubo hasn't yet drawn a kiss scene except for the 'nearly' kiss between Ichigo and Orihime.
