Yes, the count reverts back to zero if the count is in the favor of the pitcher unless the pitcher leaves injured or is traded!

MLB Rule 6.02 is pretty clear:The batter shall take his position in the batters box promptly when it is his time at bat.(b) The batter shall not leave his position in the batters box after the pitcher comes to Set Position, or starts his windup.PENALTY: If the pitcher pitches, the umpire shall call Ball or Strike, as the case may be."The official commentary on this rule says even more: "Umpires may grant a hitters request for Time once he is in the batters box, but the umpire should eliminate hitters walking out of the batters box without reason. If umpires are not lenient, batters will understand that they are in the batters box and they must remain there until the ball is pitched."Unless an umpire agrees to call, "Time" at the request of the batter, the pitcher may pitch at will, and the umpire can call a strike if the pitcher throws the ball down the middle of the plate without a batter there. If a batter refuses a request to re-enter the box, the umpire can call a strike even without a pitch.
Middle Relief Pitcher. A pitcher who goes in around the 5th - 7th inning.

The new pitcher...he is the one who actually has to stike them out, he could still walk them even if the old pitcher had 0-2 count.
It will depend on the circumstances. If this happens in the top of the ninth inning, he will get a save if he retires the remaining batters without giving up a run, but if he gives up one run to tie the game, so that the game is tied in the middle of the ninth, he will get a win if his team gets a run in the bottom of the ninth inning. No, the number of balls and strikes stays the same when the new pitcher comes in.

Pitches the ball down the middle of the plate.
Yes. However, rules state a pitcher must pitch one complete at bat before being taken out of the game (unless the pitcher gets injured and cannot continue). So, as long as the batter being pitched to is not the first batter the pitcher is facing, the pitcher may be removed in the middle of the count.
He is a good middle to late or long relief pitcher. In any other role he is either slightly below average(starting pitcher) or bad
