she does not die. she gets injured and somehow the spirit of fallenleaves helps her get better. she helps the clan in secret and it is her that saves the two apprentices from the fox/badger. she rejoins the clan when jayfeather and lionblaze stop her returning to the tunnels. she dies in the great battle to save dovewing/ivypool. she has no known kits. (i am not sure if it was a fox or a badger and i am not sure if it was ivypool or dovewing.

She was a good, loyal warrior who wanted to follow the warrior code every moment of her life. She followed the code to a fault, actually, analyzing everything that even might go against it and pondering what she should do about it. Then she found out that her existence was against the warrior code... she was half-clan... and she went berserk. On the other hand, she killed Ashfur to keep her mother's secret quiet, and she ran away from her clan. So if you're a huge Ashfur fan, then you probably should hate Hollyleaf.
