The steam is piped all accross the city to buildings that either don't have room or are much to big for their own boilers. If the Empire State building had its own boiler, there would need to be a huge smokestack at the top!

It would be unusual for any town, city, etc. not to require some type of plumbing codes, with the exception perhaps being private rural property. In Minnesota we have a state code. There is a Uniform plumbing code that is national but many states supercede it with their own code. At least for now.
