If you mean the 2004 movie by Joel Schumacher only Christine dies but it never tells how. I would say by the scenes old age, since Raoul is over 70, viewing her gravesite, discovering the Phantom's rose.

The Phantom disappears after he sets Christine and Raoul free. By the crashing of the mirrors, it is also implied that he commits suicide. He is heartbroken of his loss. He dies of a broken heart if nothing else.

The Phantom of the Opera by written Gaston Leroux the original creator, Erik (The phantom.) does die of aging illness, no one is sure how old he really was. His body was rapidly decaying from his time of his birth to his death.

Several Phantom of the Opera movies about how the Phantom dies, one them and most famous when Erik(The Phantom) is caught, the crowd kills him this is 1925 version with Lon Chaney.

That is a very good question. I don't think he did. All we know is that he left his lair with smashed mirrors everywhere. I don't think he would've died. My guess is that he went and found somewhere else to hide seeing Madame Giry led everyone to see where he had hid.
