Two flares fixes the need for unsightly o-rings that can fall off and sometimes get stuck in your ear.
They can be difficult to put in sometimes though because the flares are bigger than the plug size you want. So use lube and maybe try after a hot shower when you skin is warm and stretchy.
The difference is mostly we use spacers for our car wheels but use ear gauges for ear.

Anything under 00g should be fine in a pit, but wear double flared plugs so they don't pop out. Anything over 00g stretches don't wear plugs to the show. If you're stretched enough to wrap your lobe around the top of your ear, some people do that. That way nobody can get their hands/fingers in your holes and rip your lobe.

The best bet for removing double flared plugs, or getting them in for that matter, is to take a very hot shower for about 30 minutes to get your skin to have more "stretch". After the shower, they should pop right in. Just remember that you should take out flared plugs EVERY DAY to clean them and so that they won't get stuck in your lobes to begin with.

Not as long as you're responsible enough to clean them and care for them. Also, they aren't gauges, they're just stretched lobes.
They're called plugs, not gauges. You can paint the front or back of them but not the part touching your ear. If you do paint the outsides, you'd have to wear silicone earskins around them so that the paint doesn't touch your ears.
