Biographical Sketch (Biosketch) Defined
A biosketch is a brief summary of you or someone else's professional /educational accomplishments, publications, and affiliations--an abbreviated curriculum vitae--meant to highlight important aspects of your training, experience, and areas of interest.
In plain words...it's a "light sketch" of a persons life.

NIH has another format it uses, and while each federal agency is different, most will accept NIH form, which is also known as the PHS 398 (Google it under that).
Finally, a biosketch that is not needed for federal purposes can look pretty much like anything, I've even seen some done in narrative format with a photo for websites. Just keep it to two pages or under.
