I not sure how the first radio active element was formed but when it release radiation it also release other radioactive elements.
For example:
the element actinium, radiates and releases thorium. thorium radiates and releases francium. all of these are radioactive elements.
Atoms when unstable ,the nucleus also becomes unstable and thus are called RADIOACTIVE.For an example: radium, uranium, radon etc.... are radioactive elements.
Radiation is formed byt little pixies with ray guns. When bored they fire them at you to give you radiation poisoning.
They live in rocks called Uranium

Current understanding is as follows: hydrogen, some helium, and insignificant amounts of heavier elements, were formed during the Big Bang. Heavier elements were formed in supernovae.
By alpha, beta, or gamma decay.

What gas is Formed when ultraviolet radiation decomposes chlorinated hydrocarbon
Phosgene is a chemical agent that is formed by decomposition of chlorinated hydrocarbon solvents by ultraviolet radiation.
adaptive radiation formed mant new land plant species
Advantage radiation wcould help protect us from space debris and make our ion asphere stronger.disadvantage radiation could kill ozone and life on earth ?
The molecule formed is ozone. It is present as the ozone layer.

electromagnetic radiations are formed due to the disturbances in electric and magnetic fields which are perpendicular to each other.

Polonium-212 is formed by beta decay of bismuth-212.
In the atomic nucleus as protons and/or neutrons fall from excited states towards their ground state.
