there are actually bacteria in your bone that constantly eat it away and enzymes that constantly build it back up. When you have a break, the bacteria stop eating the bone and the enzymes have an opportunity to build it uninhibited. This actually means that where you had your break eventually there will be a mass of bone much larger than normal. When this happens, the bacteria are then reintroduced and allowed to eat at the bone until all returns to normal. This being in an average small break.

If a bone is broken, it should be aligned in such a way that the broken ends are close together. They will make new bone the way that they made bone as a fetus and a child. They will form a cartilage bridge between the ends and then replace the cartilage with new bone. The bone also over-grows and forms a callus. This is sort of like a cast would be and adds strength to the growing bones. The process takes awhile to be complete. If this bone is weight-bearing (leg or foot), a person must stay off of it for at least 8 weeks. The callus slowly is reabsorbed.

If you have wear and tear in the hip joint can the bone and cartilage grow back?

No not at present there are experiments at the moment going on in several country's where the aim is to regrow cartilage and bone in a controlled manner to repair joints but nothing has been introduced in that direction yet. one of the big problems wit arthritis is that the bone that is damaged as a result of missing cartilage tries to repair itself leading to ridges of bone forming where they do more harm then good.
