Each state has a specific form and procedure when it comes to applying for pardon. In Georgia for example you would have to fill up the APPLICATION FOR RESTORATION OF RIGHTS/PARDON and request a pardon from the Parole Board. Complete application should be filled and sent to the STATE BOARD OF PARDONS AND PAROLE!
