Hermione accidentally broke it when they were in Bathilda Bagshot's home, and Nagini attacked them. While she was apparating them, the wand snapped.

Actually it got hit by one of Hermione's rebounding spells!

He disposed of the phoenix feather one when he discovered the connection between his and Harry's phoenix one. The elder wand was won from him by Harry Potter, even though it was never truly his (or Draco's), it lay in Dumbledore's tomb until Voldemort stole it.

He broke it because it only brought trouble. He didnt need the wand and didnt want it to be in possession of any other wizard or witch because he had seen the harm the wand brought its owners.

It didn't happen in the book, mind you.
