Most mammals have two lungs, and a diaphragm. The diaphragm pulls downward the lungs, which suck air in (much like a vacuum), and the diaphragm then pushes upward on the lungs, to exhale (breathe out) carbon dioxide.

well.... the diaphragm contracts, which increases the volume of the lungs. This means that the air pressure on the inside of the lungs is lower than the air pressure outside. Then, the air goes down the trachea which branch off into the bronchii. These then branch to the bronchioles, which are on the inside of the lungs. Unlike common belief, the lungs are not like balloons, but are actually more like sponges. Anyway, at the end of the bronchioles are hundred of alveoli, which are little sacks that fill with air. These sacks are all connected to capillaries, which are very tiny blood vessels. Since the cell walls of thecapillaries are so thin, the oxygen molecules can easily move across into the capillaries from the alveoli, while the carbon dioxide moves out and back up the way the oxygen came and you exhale. BOOM! Outsmarted by an eighth grader. haha.
they pop there head out of the water for most animals. but like a dolphin it will put it's blow hole out of the water and breath like that with their face completely submerged

However, there are still a number that live exclusively in the water (such as whales, dolphins, manatees, etc) and many more that spend much of their time in water (such as beavers, seals, platypus, etc). One thing to note about aquatic mammals is they cannot get oxygen from water like fish, so they have to surface to breathe air frequently.
