You call them "President" or "Mr President".

It doesn't matter that they are past-presidents, the name carries on the same.

Wait a minute...you mean deceased as in died? Anyway, I would just address them by their full name (Mr. Reagan, Mr. Nixon, Mr. Bush, etc.)
